ADMISSION CRITERIA
Hill House offers an alternative to those who, for whatever reason, are not able to be cared for at home, but who do not wish to die in a hospital or institutional setting.
The individual must be:
- an adult (age 19 or over);
- a resident of York Region (or a family member is a York Region resident);
- diagnosed with a terminal illness; and
- deemed to have a life expectancy of less than one month.
Referrals will be accepted from attending physicians, hospitals, homes for the aged, community hospice organizations, other health care professionals or family members.
Prospective residents who are able and/or their families will be required to visit the house. They will receive an orientation package that describes the care provided and will have an opportunity to ask any questions they have. Prospective residents and their families must understand and agree to accept the level of care offered at Hill House Hospice before they are added to the waiting list.
The Executive Director will be responsible for determining whether prospective residents meet the established criteria.
